Current Covid-related Policy and Screening Questions

Whether you have tested positive for covid or not, if you have symptoms please switch to an online appointment or reschedule. If someone in your household has symptoms of illness, please wear a medical grade mask during your appointment, or switch to an online appointment, or reschedule. Whether I contract covid or a cold or flu I will need to cancel all of my patients and I am doing my best to stay healthy so that I am available for in-person appointments. Thank you so much for your help with this.

Masks are no longer mandatory in healthcare settings, however, they are still recommended in order to reduce the spread of any respiratory illnesses during our sessions.

Since masking is now optional, please note there may be both masked and unmasked patients in our waiting room. If you would rather wait in your vehicle until your appointment time, please text me when you arrive, and I will let you know when I am ready for you. All of our practitioners want to ensure you feel safe and comfortable during your time in our office.

Self-Screening Questions:

1. Do you currently, or within the last 5 days, have any of the following symptoms? Please check all that apply.

Fever and/or chills

New onset of cough or worsening chronic cough

Shortness of breath

Decrease or loss of sense of taste or smell

Runny nose or nasal congestion (not related to allergies or other known cause)

Headache (new, unusual or long-lasting)

Extreme fatigue

Sore throat

Muscle aches or joint pain (unusual or long-lasting)

Gastrointestinal symptoms (such as vomiting or diarrhea)

2. In the past 5 days, have you or anyone in your household tested positive for COVID-19? If yes, please switch to an online appointment or reschedule.

3. Is anyone in your household currently sick with symptoms of either COVID-19 or a cold or flu? If yes, please switch to an online appointment, wear a medical grade mask during your appointment, or reschedule. Please use your discretion to help keep us both healthy.

4. In the past 10 days, have you been a "close contact" of someone who currently has COVID-19 or are concerned about exposure to a known COVID-19 case in any other setting (ie. workplace or school outbreak)?

Updated Cancellation Policy:

Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in my schedule that could have been filled by another patient. As such, I require 24 hours notice for any cancellations or changes to your appointment. Patients who provide less than 24 hour notice, or miss their appointment, will be charged for their appointment.

If you wake up with new symptoms of illness on the day of your appointment, please switch to an online appointment, or if that is not possible or other unforeseen circumstances have come up and you are not able to provide 24 hours notice of cancellation, please contact me as soon as possible to discuss your individual situation.
